FREQUENTLY ASKED QUESTIONS
Below you will find answers to the most common questions about the health plans available to you as well as programs only available to Corsair employees.
Employees who are scheduled to work an average of at least 30 hours per week are eligible to be enrolled in benefits. You may also enroll your eligible dependents in any plan that offers dependent coverage. Eligible dependents may include the following:
• Your legal spouse/domestic partner
• Your children up to age 26
• Your unmarried dependent children over age 26 who are incapable of self-care because of a disability and who rely on you for support
You may only change your coverage during the year if you have experienced a qualifying life event.
As defined by the IRS, a qualifying life event change includes (but is not limited to): birth or adoption of a baby or child, loss of other healthcare coverage, eligibility for new healthcare coverage, marriage, divorce, change in child's dependent status, or state address change.
If you have experienced a qualifying life event and intend to make a change to your elections, please notify and share supporting documentation of your life event to HR@corsair.com. You will process your enrollment changes on your Benetrac account and have 31 days from a qualifying event to amend your benefit elections.

Please reach out to HR@corsair.com, and describe your claim and the issues you are encountering with your provider or insurer. We can assist and follow up with your carrier, if necessary.
First, we recommend registering and downloading the mobile apps for each carrier you are insured with. United Healthcare, Kaiser, and Guardian have digital ID cards through their mobile app that you can use to share with your provider. Each carrier is available on iOS and Android. We also encourage you to explore the carrier resources pages, you can find additional helpful information along with programs and resources available to you and your family members.
When you first enrolled in UHC, you should have received your ID card and instructions in the mail to register online at myuhc.com. In the event you have lost or misplaced your physical ID card, you may request a new one online.
Kaiser (California-only)
When you first enrolled in Kaiser, you should have received your ID card and instructions in the mail to register online at kp.org. In the event you have lost or misplaced your physical ID card, you may request a new one online.
Guardian does not distribute physical ID cards. Register online at guardianlife.com to download your digital ID card. For Guardian, please note that ID cards are not required for you to receive services. Providers can confirm coverage with your Social Security Number. Any dependents on your plan can also use your Social Security Number to get care.

Depending on the volume coverage of your enrollment, you may be subject to Evidence of Insurability (EOI) rules and be required to submit an EOI. You can find and complete the online EOI form through your Guardian Anytime account or www.GuardianAnytime.com/eoi.
Employees who previously declined coverage during their initial new hire enrollment must submit EOI for any coverage amount when enrolling in a voluntary benefit. Learn more about EOI eligibility rules and the EOI submission process.
If you haven't already, register an account with Navia and request a new card. Corsair's Navia Employer Code is CMM. If you encounter issues acquiring a replacement card, please reach out to HR@corsair.com.
